The front room is a cat-free cafe serving coffee, beer, wine and sake with sweet and savory snacks, and it is open to everyone; large windows look into the connected cat room. The cat room houses 10 to 12 rescue cats from the Alley Cat Project and Regional Animal Services of King County, most of them adoptable, alongside a few permanent residents. NEKO operates as a rescue and handles adoptions in house. Visits to the cat room are 45-minute paid sessions with up to eight guests, best booked online, and private sessions for small parties are available. All ages are welcome, with adult supervision rules for children, and NEKO merchandise is sold in the cafe.

Opened in December 2015 as Seattle's first cat cafe, it has a coffee shop serving espresso drinks, teas, Italian sodas and cat-shaped pastries, with a separate cat lounge that typically holds 18 to 25 cats. The cats come from Regional Animal Services of King County and the Seattle Animal Shelter, and the cafe's nonprofit arm, the Meowtropolitan Foundation, places them for adoption; some stay on as lounge ambassadors before becoming adoptable. Lounge visits are paid 50-minute sessions that are best reserved online, with a few walk-in spots. Children must be 6 or older, and Friday to Sunday evening sessions are for adults only. Drinks bought in the cafe can be taken into the lounge. The cafe also runs cat yoga, cat Pilates, sound baths, paint nights and memberships.

Around six resident cats roam the maze of shelves, nooks and reading corners, and visitors can pet them and find them napping among the books. The cats live at the store full time and are not available for adoption. There is no entry fee or booking: the shop is open to everyone every day, and the cats are part of an ordinary visit to browse thousands of secondhand and collectible titles. The store has been trading in Seattle since 1987 and also buys books from the public on weekdays. It is not a cafe, so there is no food or drink, and the cats may be asleep or out of sight at any given time.
